What the recommended pregnancy weight gain ranges are, where the pounds actually go, and the weekly rate worth checking against.

Quick answer: Recommended total pregnancy weight gain depends on your BMI before you conceived. For a BMI of 18.5 to 24.9 the usual range is 25 to 35 lb (11 to 16 kg). Overweight is 15 to 25 lb, underweight 28 to 40 lb, and obese 11 to 20 lb. Most of the gain arrives after week 13.

The number on the scale during pregnancy is one of the least useful things to watch daily and one of the more useful things to watch monthly. A calculator turns it into a trend rather than a verdict.

The ranges, and where the weight goes

Targets are set by pre-pregnancy BMI, so that is the first thing to work out. Someone 5 ft 5 in and 140 lb before pregnancy has a BMI of 23.3, which lands in the 25 to 35 lb band. At 5 ft 4 in and 158 lb the BMI is 27.1 and the band drops to 15 to 25 lb. Two people eighteen pounds apart, ten pounds of difference in the target.

It helps to know what the weight is made of. A rough breakdown at term: baby about 7.5 lb, placenta 1.5 lb, amniotic fluid 2 lb, extra blood 4 lb, uterus 2 lb, breast tissue 2 lb, and stored fat 6 to 8 lb. That adds up to around 26 lb, which is why the normal-BMI band sits where it does. Only the last item is really within your control.

The first trimester is nearly flat

Expect 1 to 4 lb across the first 13 weeks. Plenty of women gain nothing at all, and some lose a little to sickness. The baby weighs about an ounce at week 13, so there is not much there to gain weight for yet.

A weekly rate you can check against

After week 13, roughly 1 lb a week (0.45 kg) for a normal starting BMI. Run it forward: 27 weeks at 1 lb, plus 3 lb early, lands on 30 lb, right in the middle of the band. Starting from a BMI of 27 the rate is nearer 0.6 lb a week, which over the same 27 weeks plus 2 lb early gives about 18 lb.

When the number stops mattering

These ranges are population guidance rather than a personal prescription, and the UK has no official target at all. NHS advice is to eat normally and drop the eating-for-two idea, because the extra energy need is small: nothing additional for the first two trimesters and about 200 calories a day in the last one. That is a banana and a yoghurt, not a second dinner.

I gained 8 lb in the first trimester. Is that a problem? Not on its own. Early gain is often fluid, and appetite in the first weeks is unpredictable. It only matters if the weekly rate stays high through the middle months. Raise it at your next appointment rather than trying to correct it by cutting back.

I have lost weight instead. Should I worry? Losing a few pounds early alongside nausea is common. Persistent loss, or losing more than about 5% of your starting weight, is worth a call to your midwife or doctor, mainly to rule out sickness severe enough to need treating.

Does it work for twins? Yes, if the calculator has a twins setting. The targets rise across every BMI band, and the gain is expected earlier, because twins usually arrive before 38 weeks.

Can I diet if I have already gained too much? Deliberate weight loss is not advised during pregnancy. The usual approach is to slow the rate rather than reverse it, keeping meals steady and adding walking. Ask your midwife before making any large change.
