Quick answer: Visceral fat sits around the organs rather than under the skin, and waist measurement predicts it far better than weight does. A waist over 40 inches for men or 35 inches for women marks the higher risk band whatever the BMI says. The simplest single check is a waist-to-height ratio above 0.5.
Two people can weigh the same, stand the same height and carry their fat in completely different places. The scale cannot tell them apart. A tape measure can.
Why the scale misses this entirely
Take two men at 5 feet 10 and 180 pounds. Both have a BMI of 25.8, so both land in the same box on the same chart. One has a 33 inch waist and carries his weight through his legs and shoulders. The other has a 40 inch waist. Their metabolic risk is not remotely the same, and BMI has no way of saying so.
Visceral fat is the fat packed around the liver, pancreas and intestines. Subcutaneous fat, the kind you can pinch, behaves differently. The visceral kind is the one linked to metabolic problems, which is why waist circumference has stayed useful long after BMI started drawing criticism. We go further into that in body fat versus BMI.
Waist-to-height ratio in one line
Keep your waist under half your height. That is the whole rule, and it works in any unit as long as both numbers use the same one. A man of 5 feet 10 is 70 inches tall, so his target is a waist under 35 inches, and a 40 inch waist puts him at 0.571. A woman of 1.63 m with an 84 cm waist sits at 0.515, just over, against a target of 81.5 cm.
The useful thing about the ratio is that it needs no separate thresholds for men and women or for different builds. A 6 foot 4 frame gets more allowance than a 5 foot 2 one, automatically.

Most people measure in the wrong place and get a flattering answer. The standard site is the midpoint between the bottom of the lowest rib and the top of the hip bone, usually an inch or so above the navel, not down at the trouser waistband.
Stand up, feet together, arms at your sides. Put the tape directly on skin, horizontal all the way round, snug without compressing. Breathe out normally and read it at the end of the exhale rather than holding your breath in. Measure at the same time of day each time, ideally in the morning before eating, since a large meal can move the number by an inch on its own.

You need height, weight, waist circumference, age and sex. Waist is the input doing the heavy lifting, so measure twice and use the average instead of guessing from your jeans size. Trouser sizes commonly run two to four inches under a real tape measurement, which explains a lot of the surprise people report at the result.
What the output is and is not
It is an estimate built from body measurements, not an imaging result. The reference standard for visceral fat is a CT or MRI scan, with DEXA a step behind. An estimator puts you in the right band and, more usefully, tracks the direction of travel over months. Treat a single reading as a rough position and the trend as the real information. For a second angle on the same question, the home body fat measurement methods have similar strengths and limits.
Common questions
Can you be thin and still have high visceral fat? Yes, and it is common enough to have a nickname: thin outside, fat inside. A normal BMI paired with a waist-to-height ratio over 0.5 is exactly that pattern. It turns up more in people who have lost muscle over the years rather than gained much weight.
How quickly does visceral fat come down? Generally faster than subcutaneous fat, which is the one genuinely encouraging thing about it. Waist measurements often move before scale weight does in the early weeks of a calorie deficit, which is a good reason to keep measuring rather than only weighing.
Do sit-ups reduce belly fat? No. There is no mechanism by which working a muscle burns the fat lying over it. Sit-ups build abdominal muscle, worth doing for other reasons, but waist reduction comes from overall energy balance.
Should I take the result to a doctor? If your waist is over 40 inches as a man or 35 as a woman, it is worth raising at your next appointment, particularly alongside blood pressure and blood glucose readings. The estimator is a prompt for that conversation, not a diagnosis.
